Office Information

OFFICE VISIT
We will try to expedite your office visit and avoid delays. To do so, we ask that you bring several items to your first visit:

• All old records (especially operative reports), x-rays, CT scans, MRI’s.
• Always bring the actual films if available
  (as opposed to the ‘reports’).
• Contact information (full address) of referring physician and primary care doctor.
• Shoulder problem
• tank top or jog bra
• Elbow problem
• short sleeve shirt

For a new patient’s initial visit, Dr. Simovitch will require new or at least recent x-rays of the affected extremity.Please be aware that new patients or those who require x-rays may be asked to come in early to obtain these important studies.
MEDICAL RECORDS/FORMS
It is best for you to request your medical records (including office notes, operative reports, laboratory data, and intra-operative pictures) in advance of your initial visit and hand carry them with you as opposed to having them mailed to us.

In order to expedite your visit, we encourage you to printout the form titled NEW PATIENT FORM under the link PATIENT FORMS, fill it out at home and bring it with you to your initial visit. Please present it upon checking in at the front desk.
INSURANCE

Please bring a copy of your insurance card with you. If you are out of our network, we will make every effort to work with you and facilitate your care with us. We also accept out of network benefits and self-pay. We are dedicated to helping the patients of South Florida receive exemplary care.
SURGERY

Inpatient surgeries can be done at Good Samaritan Hospital, Palm Beach Gardens Hospital and Jupiter Medical Center while outpatient surgeries are done at the Northpoint Surgery & Laser Center (W. Palm Beach) or the Jupiter Outpatient Surgery Center. We will make every effort to accommodate your needs when scheduling a surgery.
Please do NOT eat or drink anything after midnight on the night before your surgery. Also, if you are having shoulder surgery, do not place deodorant on the morning of your surgery. Please direct any questions to Diana Kupko, medical assistant to Dr. Simovitch, at (561) 694-7776, extension 4110.
